What to do at the time of discharge & How to submit a Claim?
The Insured at the time of discharge should authenticate all the bills by signing each on of them. The Insured should submit all the following documents and reports in original (if collected by him) within 7 days after discharge
Duly signed Claim Form.
Hospital Bill / Payment Receipt.
Discharge Summary / Card. All investigation reports.
Bills and receipts for investigations along with Doctors prescriptions.
Bills of pharmacy, Laboratories along with corresponding original prescription duly endorsed by the treating doctor.
Reimbursement of Pre/Post Hospitalization:
Relevant medical expenses incurred before admission and after discharge from the hospital will be reimbursed as per the policy. Prescriptions and bills/receipts of such services should be submitted to PHS along with duly signed claim form.
In Network hospital Members will have an advantage of getting cashless hospitalization.
For extending these services to the members PHS has created for you, a network of providers consisting of reputed hospitals & nursing homes.

Note:
The cashless access in PHS network of hospitals is merely a facility extended to you by PHS under contract with your Insurance Company.
PHS shall not responsible for availability, quality and clinical outcome of the treatment. Selection of a network or out side of the network hospital is a prerogative of the insured.
In case of loss of PHS ID card, it will be issued at an additional charge of Rs. 50 per card.
